- These divisions also work to coordinate individual service needs
Pottawattamie County Targeted Case Management:
To be eligible, one must have a diagnosis of Mental Retardation, Chronic Mental Illness, or a Developmental Disability. This is a Medicaid funded program; therefore, the person has to be receiving Medicaid benefits in order to access this service. Targeted Case Management is a service, which involves the person having goals they wish to achieve, and services to help them achieve those goals. The case manager is a key member of the individual's team in helping them plan and access services. They continue to monitor those services to assure the individual is getting what they expected from the service. The Case Manager acts as an advocate to help the individual achieve their desires. For more information regarding the Pottawattamie County Case Management program, call the Intake Supervisor at 712.328.5812.
Social Work Division:
This division is a County program to assist individuals with some of the same services listed above. One does not need to be receiving Medicaid benefits to access this program. The social workers in Pottawattamie County also work with individuals who are receiving services in Intermediate Care Facilities for the Mentally Retarded to assess their continued level of service need. They work with individuals with disabilities who have not yet qualified for Social Security Disability or who are in the process of applying for certain entitlements. Many of the people being served by the social workers have an immediate need causing a crisis in their lives. The social workers help individuals maneuver the sometimes-confusing mental health and social service programs available to them. For more information regarding this service, call the Intake Supervisor at 712.328.5812
